Over $600, bring your signed ticket, ID and a completed claim form.
Full checklist (5 items)
- The original winning ticket, signed on the back with your name, address and phone number
- For a prize of $600.01 or more, or a ticket more than 180 days old, a completed and signed claim form with your Social Security number
- A photo ID, such as a driver's license or a PennDOT non-driver photo ID, with the same address as your claim form
- Any claim ticket the retailer's terminal printed when it checked your ticket
- For a group win, a list of every member's name, address and Social Security number, and an IRS Form 5754 from each

- Jackpots and annuity prizes
- Jackpots and top prizes of Powerball, Mega Millions and Powerball Xs & Os, and Millionaire for Life prizes paid yearly, are claimed in person at Lottery Headquarters in Middletown, which is otherwise closed to the public. For questions about a claim, call the Claims Department at 800-692-7481.
- Choosing cash or yearly payments
- Powerball and Mega Millions jackpot winners, and Millionaire for Life's top two prize winners, have 60 days from the day they claim to choose one cash payment or yearly payments.
- Retailers
- Any Pennsylvania Lottery retailer can pay prizes up to $2,500 by cash, check or money order. If one can't, try another retailer or claim by mail.
- Claim form
- A prize of $600.01 or more needs a claim form, and so does any winning ticket more than 180 days old, whatever its prize.
- Group winners
- One member of the group signs the ticket and claims for everyone, attaching a list of every member's name, address and Social Security number and an IRS Form 5754 from each. A group claim is final. Separate checks are possible only for a prize of $5,000 or more where each share is $500 or more.
- Tickets bought in Pennsylvania
- You don't have to live in Pennsylvania to win, but the Pennsylvania Lottery can only pay prizes on tickets it sold, multi-state games included.
- Quick Cash prizes
- Cash 5's Quick Cash prizes must be claimed within one year of the day the ticket was bought, not the drawing.
- Prizes won online
- Prizes on games played or bought online at palottery.com are usually paid straight into your Lottery account.
