Over $600, bring your signed ticket, ID and a completed claim form.
Full checklist (6 items)
- The original winning ticket, signed on the back
- A completed and signed Lottery claim form, for a prize over $600
- Signed photo ID, such as a driver's license, Mass ID, passport, work ID or military ID
- Proof of your Social Security number, such as a signed Social Security card, a W-2 or SSA-1099, a pay stub, or a driver's license that shows it
- By mail, copies of your signed photo ID and proof of Social Security number for a prize of $601 to $50,000, and keep copies of your claim form and both sides of your ticket
- For a lifetime prize, a certified copy of your birth certificate, unless you take the cash option

More Claim Rules
- Prizes won online
- Prizes under $601 won on Mass Lottery Online go straight into your player account. Prizes of $601 to $49,999.99 go through the online claim process and are usually in your account within 24 to 48 hours. Prizes of $50,000 or more must be claimed at Lottery headquarters in Dorchester, and can't be claimed by a trust.
- Cashing a ticket in the app
- A store ticket that wins $601 to $5,000 can be cashed in the Mass Lottery app: scan it and the prize is paid to your bank account. The ticket can't have drawings still to come, and prizes from separate drawings can't be added together to reach $601.
- Claiming by trust
- To claim a prize through a trust, contact the Lottery's Legal Department first, at trustclaim@masslottery.com or 617-639-2599.
- Regional office limits
- The Lawrence, New Bedford, West Springfield and Worcester offices don't take claims by a trust, company or corporation, by power of attorney, with a sign-over form, or for an altered or damaged ticket, nor Powerball claims of $100,000 six months or more after the drawing.
- Debts owed to the state
- Past-due child support, unpaid state taxes and other debts owed to the Commonwealth are taken out of a prize before it's paid.
- Public record
- A winner's name, city or town, photo, prize, claim date and game are public record, and the Lottery may photograph winners to publicize their prizes.
