Full checklist (5 items)
- The original winning ticket, signed, with your name printed on the back
- For a prize of $600 or more, a completed and signed Claim Form
- For a prize of $600 or more, the Pay to Bearer or File Claim receipt a retailer prints when it checks your ticket, signed
- A valid photo ID with your current address, such as a driver's license, state ID card, passport or military ID
- For a Classic Lotto jackpot, a Cash Option Election Form saying whether you take cash or 30 yearly payments

More Claim Rules
- When you can claim
- Pick 3, Pick 4 and Pick 5 prizes can be claimed the day of the drawing. Classic Lotto, Rolling Cash 5, Powerball and Mega Millions prizes can be claimed from 6 a.m. the day after the drawing.
- Racinos and Super Retailers
- Besides regional offices, Ohio's seven racinos cash prizes up to $25,000, and more than 30 Super Retailers cash prizes up to $5,000, the same day. Racinos are for players 21 and older unless the lottery's list of cashing locations says otherwise.
- Mobile cashing
- The Ohio Lottery app's mobile cashing pays a single win of $50 to $25,000 into your bank account, using your MyLotto Rewards account. Classic Lotto tickets that win with KICKER, and tickets with several wins, can't be cashed this way.
- Jackpots and yearly prizes
- Jackpot winners of Classic Lotto, Powerball and Mega Millions, and winners of prizes paid yearly, call their nearest regional office to make an appointment to claim. These prizes can't be claimed with the online claim form.
- Debts owed to the state
- Before paying a prize of $600 or more, the lottery checks for unpaid child support, spousal support and debts owed to the state, and pays those out of the prize first.
- Group wins
- Winners sharing a prize contact their nearest regional office to claim it.
- Claiming through a trust
- A winner's name and address are public records. When a prize is claimed through a trust, the names of the trust's owners are kept confidential unless they agree in writing to be named.
- Tickets bought in Ohio
- Prizes on tickets bought in Ohio can only be claimed in Ohio.
- Claiming by mail
- Send a mailed claim by registered or certified mail and keep copies of everything you send: the lottery isn't responsible for lost mail.
