    Prizes up to $49,999 can be claimed by email to claims@lottery.ok.gov. Send photos of the front and back of the signed ticket, filled in on the back, and for a prize over $600, a photo ID and the completed claim form, plus proof of your Social Security number over $5,000. Keep the original ticket. Prizes of $50 or less should be cashed at a store.
    Debts owed to the state
    Before paying a prize, the lottery takes out any delinquent debt the winner owes to the Department of Human Services, such as child support, to the Oklahoma Tax Commission or to the Oklahoma Employment Security Commission.
    Your name
    The details you give to claim a prize over $600 are kept confidential, except your first name, your city and the prize amount, which can be released. When a trust claims a prize, the details of everyone sharing it are kept confidential.
    Group wins
    People sharing a ticket fill in the claim form's joint ownership section: how many claimants there are, and each one's share of the prize.
    Cash or yearly payments
    Millionaire for Life winners of the top two prizes choose cash or yearly payments within 60 days of becoming entitled to the prize, and the choice can't be changed.
    Damaged tickets
    A damaged ticket may be held until its claim period ends before it's paid.
