Claim an Iowa Lottery prize $600 or less at any retailer, a regional office or the Iowa Lottery Main Office in Clive, over $600 to $250,000 at a regional office or the Iowa Lottery Main Office in Clive and over $250,000 at the Iowa Lottery Main Office in Clive, within 180 days of the drawing. Pick 3 and Pick 4 prizes have 90 days. Prizes up to $250,000 can be claimed by mail.

Full checklist (5 items)
- The original winning ticket, with your name and address filled in on the back and signed. A parent or guardian also signs for a winner who is a minor
- For a prize of $100 or more, a completed Winner Claim Form and IRS Form W-9, or Form W-8BEN if you aren't a U.S. citizen
- For a prize of more than $600, a valid government-issued photo ID, such as a driver's license, state ID, military ID, passport or permanent resident card. Send a legible copy if you claim by mail
- If you aren't a U.S. citizen, a copy of a government-issued photo ID from your country of citizenship. A permanent resident brings their Permanent Resident Card or Social Security card, or more tax may be withheld
- For a group claim, IRS Form 5754 listing everyone who shares the prize

More Claim Rules
- Prizes over $250,000
- A prize over $250,000 is claimed in person at the Iowa Lottery's main office in Des Moines. Winners are strongly encouraged to make an appointment first.
- Debts owed
- Iowa law requires the lottery to take any debts a winner owes to government agencies out of a prize before paying it. The winner gets what's left.
- Cash or annuity
- A jackpot winner has 60 days after claiming to choose cash or the annuity, and the choice is final. With no choice made in that time, a Powerball, Mega Millions or Lotto America jackpot is paid as an annuity.
- Group wins
- A group can be paid through one person or an entity such as a trust. One person signs the ticket for the group, says on the Winner Claim Form that they're claiming for others, and files IRS Form 5754 listing every winner's address and tax ID, so the prize and the tax withheld are split correctly. The lottery suggests not signing the ticket until the group has decided how to claim it.
- Winners under 21
- You must be 21 to buy a ticket, but someone younger can win and claim a prize on a ticket a buyer 21 or older gave them. A parent or guardian also signs the ticket, and the claim form, for a winner who is a minor.
- Disputed tickets
- If a retailer's terminal rejects your ticket or the retailer disputes your claim, you can send the claim to the Iowa Lottery to review.
