- The original winning ticket, with your name, address and phone number filled in on the back, and signed
- For a prize of $600 or more, a completed and signed New Hampshire Lottery Claim Form
- For a prize of $600 or more, a current photo ID: a driver's license or non-driver ID from a U.S. state or Canadian province, a passport or a military ID. Send a copy if you claim by mail
- For a prize of $600 or more, proof of your Social Security number, such as your signed Social Security card, a driver's license or health insurance card that shows it, or a pay stub from the last 30 days. Or fill in a W-9 at the Lottery. Send a copy if you claim by mail

- Prizes won online
- Prizes on games bought with NH iLottery are claimed online up to $20,000. Under $2,000 they go straight into your account. From $2,000 to $20,000, submit a claim under Tax Reported Prizes in your account, and the prize is deposited within 3 to 5 business days. Over $20,000, start the claim in your account, then claim at Lottery Headquarters or by mail.
- Cash or yearly payments
- Powerball, Mega Millions and Tri-State Megabucks jackpot winners have 60 days after filing their claim to choose one cash payment instead of yearly payments, and the choice is final. A Megabucks jackpot is paid yearly if no choice is made. Millionaire for Life top and second prize winners make a final choice of cash or yearly payments within 60 days of becoming entitled to the prize.
- Your name
- Claims are public records, and the Lottery Commission's practice is to release winners' names and hometowns. Street addresses, phone numbers and Social Security numbers are kept confidential. A trust can claim a prize through its trustee, who shows their own photo ID.
- Sign your ticket
- A ticket belongs to whoever holds it until it's signed, so sign the back as soon as you buy it. Someone claiming for the winner brings the winner's photo ID, or a copy of it, and their own.
- Child support owed
- Before paying a prize of $2,000 or more, including prizes won online, the lottery checks for past-due child support and pays it out of the prize first, as state law requires.
