- A current government, state or tribal photo ID, such as a driver's license, state ID card or passport
- For a prize of $600 or more, your Social Security number or Individual Taxpayer Identification Number, or a completed IRS Form W-8BEN
- For a group win, a court order naming everyone who shares the prize, and a claim form for each of them

More Claim Rules
- Retailers
- Retailers may pay prizes up to $599, or only as much as their normal cashing practices allow. The lottery suggests claiming prizes up to $599 at a retailer.
- Visiting a Lottery office
- Arrive by 3:30 p.m. to leave time for processing, and by 2:00 p.m. for a prize of $50,000 or more. Call 608-261-4916 before bringing a prize of $200,000 or more, and Powerball, Mega Millions and Megabucks jackpot winners call first to make an appointment.
- Debts owed to the state
- Before paying a prize of $600 or more, the lottery checks with the Department of Revenue for delinquent state taxes, child support or other debts owed to the state, and takes them out of the prize.
- Group winners
- Only one person can sign and claim a ticket. To share a prize, a group needs a court order naming everyone who shares it, and the claim is handled only at the Madison office.
- Trusts and companies
- Only a person can claim a prize. Trusts, companies, nonprofits and other entities can't play or claim.
- Winner Wagon
- The lottery's Winner Wagon visits stores around the state and pays prizes under $200,000, by check. Its stops are listed on the How to Claim a Prize page.
- EZ Match prizes
- Any retailer can pay a Megabucks EZ Match prize right after you buy the ticket, and gives you an exchange ticket for the drawing. EZ Match prizes must be claimed within 180 days after the ticket is printed.
- Advance Play tickets
- You can claim a prize on a ticket with drawings still to come. You get an exchange ticket for the drawings left.
- Claiming by mail
- Mail the signed ticket and a claim form, and keep a copy of both sides of each. Certified or registered mail is suggested: the lottery isn't responsible for tickets lost in the mail. A mailed claim must be postmarked within the 180 days and arrive within 14 days after them.
