- The original winning ticket, signed on the back with your name, address and phone number
- For a prize of $600 or more, a completed Winner Claim Form
- For a prize of $600 or more, a photo ID, such as a driver's license or passport. Send a copy if you claim by mail
- For a prize of $600 or more, your Social Security card, a W-2 or another government form showing your name and full Social Security number. Send a copy if you claim by mail

- Sign your ticket
- An unsigned ticket belongs to whoever holds it, so sign the back of a winning ticket right away and add your name, address and phone number.
- Who can claim
- Only the person who signed a winning ticket can claim its prize, and trusts and other entities can't claim prizes. If several people sign a ticket, the Wyoming Lottery splits the prize evenly among them.
- Claiming in person
- Claims at the Wyoming Lottery's Cheyenne headquarters are by appointment only: call 307-432-9300 first. Prizes of $50,000 or more must be claimed there in person.
- Your name
- The Wyoming Lottery honors every winner's request to stay anonymous, and never shares a player's information publicly without the winner's permission.
- Cash or yearly payments
- Powerball and Mega Millions jackpot winners have 60 days after their claim is validated to choose one cash payment instead of yearly payments; without a choice, the prize is paid yearly. Millionaire for Life's top two prize winners choose between cash and payments for life within 60 days of becoming entitled to the prize. A choice once made is final.
